Personal Information

Fill in your name, address, telephone, federal Employer Identification Number (EIN) , state employer identification number (if applicable), and social security number. If you have not received your EIN yet, you may enter "Applied For." None of this information is required; however, we recommend that you fill it in for your household employee's pay stub and your reports.

Employer-Paid State Unemployment Tax Rates

Your employer-paid unemployment tax rate and wage base is entered under the "General State" tab. For some states, the rates may be entered under the "Special State" tab. The rates for new employers can be found on your state's labor tax web site. Employers are typically notified by the state of changes to the rates.
